HOW TO REGISTER

Welcome to Glastonbury Health Centre

We are pleased to accept registration requests from patients who are living within the area indicated in the map below:

Please see our practice boundary and check your postcode is within our catchment area.

Anyone living within our practice boundary is welcome to apply to register with us. Prospective patients are asked to bring their medical card to the surgery (don't worry if you can't find it, we can give you a form to fill in).

How to register with our Doctors

Please Contact the Surgery to request some registration forms. Remember to let us know how many people are registering and if they are adults/children so we can send the correct forms to you.

Change of Personal Details

Please inform reception if you change your name, status, address or telephone number, especially mobile telephone numbers. You will be asked to complete a form to ensure that your records contain up-to-date information or you can complete our Change your Contact Details form.
