If you are still having issues, and wish to submit a prescription request, please use the form below. Prescriptions will usually be issued within 3 working days.

Please note that this route is a secure submission to the surgery but will not bring up your personal details or details of your medication. You will need to enter details of your medication onto the form including the dosage and quantity you wish to order.
